Please ensure that you use these checklists when presenting the Southeast Community Care by Arcadian Health product in a sales setting. It will protect you against scrutiny from CMS secret shoppers. Below are the checklists for the states of South Carolina & Georgia for the Plus, Dual-SNP, and Enhanced plans. The checklists have been created and approved by CMS to provide additional support and guidance to agents while conducting an in-home presentation. It identifies key elements which need to be discussed and allows for the beneficiary to acknowledge that this information was presented.
This piece has been designed for utilization during an in home appointment; however, in the event an agent wishes to utilize the checklist as a guide during a group sales event it must only be utilized by the agent – NO beneficiary signatures of acknowledgement may be obtained at group sales events.
